There are separate Patient Connect logins depending on the location of
treatment.
Boca Raton Regional Hospital (General):
Boca Raton Regional Hospital is pleased to introduce its new Patient Connect
Portal. Beginning Tuesday, August 20th, ALL patients need to re-register
as a FIRST TIME USER in order to access their records.
If you need help signing up for Patient Connect, please call
800-287-9085 for assistance.
Patient Connect also allows for a patient to designate a caregiver, such
as a husband, wife or parent, to have access to your medical information.
Given the growing percentage of individuals who are considered caregivers
for another person, we urge you to take advantage of this option if it
is beneficial to your particular circumstances.

Baptist Health Medical Group - North Physician Offices are pleased to introduce
their new Patient Portal, MyHealthRecord.com. Beginning August 20th, patients
can sign up for this new portal below.
